Short Description:Under the general supervision of the Director, the Pharmacist will be responsible for having a working knowledge of all areas of pharmaceutical services provided by Select.

Follows all facility guidelines outlining standards of personal appearance, attendance and punctuality.

Maintains adequate drug supply at pharmacy at all times. Orders appropriate quantities on a timely basis. Follows established procedures for procurement of medications. Follows established procedures for procurement of medications on an emergency basis. Completes annual inventory process and forwards results to the Business Office. Complies with all Federal and State Pharmacy laws, rules, and regulations. Oversees monthly removal of outdated medications and returns them to the manufacturer. Documents all recalled drugs. Maintains an available stock of antidotes and other emergency drugs, both in the Pharmacy and in patient care areas. Participates in the development and subsequent updating of a hospital formulary or drug list. Provides hospital formulary or drug list to the professional staff that use it.

Correctly prepares all drugs and chemicals. Prepares, sterilizes, and labels parenteral medications and solutions that are manufactured in the hospital. Ensures that individuals who prepare or administer large volumes of parenterals have performed competency assessment to do so. Ensures that all sterile products prepared in the hospital are properly labeled and include appropriate expiration dates.
Correctly fills and dispenses all medication. Reads and interprets drug orders. Fills and labels all drug containers issued to the departments/services. Fills drug orders. Fills stat orders according to protocol. Checks orders prepared by supportive personnel. Ensures strict control and accountability for drugs dispensed to patients or floor stock. Dispenses of non-parenteral products. Supplies self-medications for appropriate patients.
Correctly records and documents all dispensed drugs. Provides written guidelines to ensure that all pharmaceutical requirements are met. Maintains records of the transactions of the Pharmacy as required by applicable law, and as necessary to maintain adequate control and accountability of all drugs. Fills and checks unit dose charts. Credits patients for drugs returned to the Pharmacy. Submits patient charges to the Business Office.
Ensures the cleanliness and order of the Pharmacy as well as checks and records refrigerator temperatures.
Conducts monthly inspections of patient care units and other areas of the hospital where drugs are dispensed, administered, or stored. Inspects assigned Pharmacy areas at least monthly.
Assures adequate control and documentation of controlled substances. Issues controlled substance and controlled substance records to patient care areas and maintains records as required by law and Pharmacy policies and procedures.
Replaces any drugs removed from emergency carts and charges patient for drug use. Replaces drugs removed from floor stock. Issues drugs and supplies to other departments.
Reviews pharmaceutical policies and procedures for the purpose of establishing their consistency with current practices within the hospital.
Maintains a means of identifying the signature of all practitioners authorized to use the pharmaceutical services as well as a listing of the Drug Enforcement Administration numbers.
Develops and participates in new employee training and education programs. Participates in orientation, education, and training programs provided for the Pharmacy staff. Provides for the educational needs of Pharmacy staff and other healthcare professionals.
Correctly informs and instructs patients on any drugs to be used after they are discharged. If necessary, provides written materials to patients for medication education.
Maintains confidentiality of patient/medical staff information. Maintains computer systems and database(s). Enters the charges into the computes. Runs billing functions as required.
Participates in quality assurance programs that relate to drug utilization and effectiveness. Obtains approval of medication usage criteria through P&T. Participates in and documents medication use review activities.
Participates in all meetings of the Pharmacy and Therapeutics Committee and implements the decisions of that committee throughout the hospital.
Participates in patient drug therapy monitoring. Monitors patient drug therapy regimens for contraindication, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose. Detects and reports suspected adverse drug reactions. Provides clinical consultation and clarification to practitioners. Supports the Pharmacy s Econotherapeutic efforts.

Accepts responsibility for ensuring and supporting accreditation through JCAHO. Promotes an environment of continuous quality improvement. Develops and empowers employees to take responsibility for decision making and problem solving appropriate to their level of expertise. Counsels and evaluates employees in a timely fashion according to hospital policy.
